B. Email or mail your application

You will need to have the Acrobat Reader installed in order to view it. The free Acrobat Reader may be downloaded from Adobe Systems Inc. at: http://www.adobe.com. Versions of Acrobat Reader run on DOS, Windows, Macintosh and Unix systems.

You can complete the application online, then print. After printing, mail the application along with all the documents mentioned in the application.

The application cannot be saved to your computer. (However, if you have a full version Adobe Acrobat, you can save the content you fill in to the form.)
